AFTER YOU SUBMIT YOUR REQUEST:

  • You will need to pay your deposit within one business day, unless we tell you otherwise.
  • We will send you an email with instructions--please watch for it. 
  • Our response may end up in your spam folder.
  • Remember, you are not officially on our schedule until your appointment deposit is paid.

















I understand that within ONE business day after submitting this request, I will need to pay a nonrefundable deposit in the time frame specified, or else my appointment will be cancelled in order to keep the schedule open for others.
